Lawsuits against a county. They paid for car repairs, but now ecnores my lawyer request to settle. Its been ov?

Firetruck came up behind me with lights on. I pulled over to the side. The truck made turn too short and got caught on my rear of car and draged me into road before releasing with a jolt.I have since had new medical issues not in accident problem. Yet, now how can a lawyer file a suit on medical problems from just the accident. I was told I need surgery to correct but because i had heart problems after accident my lawyer says it is gonna be too hard to sepperate accident trouble from Heart troubles, I do not understand because I still and will continue to have the problems I had from accident. My lawyer says because of heart trouble I will never be able to get operation to correct issues from accident. So, what can I do? I get impression the lawyer just wants me to go away now. The only thing I get now is medication for the pain and I have been paying for that myself.
